RESUMO

Objetivo: realizar a tradução e equivalência cultural e linguística para o Português Brasileiro do Eating and Drinking Ability Classification System (EDACS). Método: realizou-se a tradução do EDACS para a língua portuguesa por duas fonoaudiólogas bilíngues e especialistas em disfagia. As duas traduções foram comparadas entre as próprias fonoaudiólogas, sendo as incompatibilidades discutidas entre si e decisões tomadas por consenso. Após o instrumento traduzido, este foi enviado para uma terceira fonoaudióloga, brasileira, bilíngue, residente nos Estados Unidos, para que a retrotradução para o inglês fosse realizada. A versão inicial do instrumento e a retro tradução foram confrontadas entre si, sendo as discrepâncias analisadas, discutidas e definidas por consenso. Resultados: os processos de tradução e adaptação cultural requereram maior esforço na definição da nomenclatura das consistências utilizadas e não trouxeram modificações com relação à estrutura da escala original. Conclusão: realizou-se a equivalência cultural do Sistema de Classificação das Habilidades do Comer e Beber ­ EDACS-PT/BR para o português brasileiro.


Objective: to perform the translation and cultural equivalence to Brazilian Portuguese of the Eating and Drinking Ability Classification System (EDACS). Method: EDACS was translated into Brazilian Portuguese by two bilingual speech language therapists, specialists in dysphagia. The two translations were compared by the speech therapists, the incompatibilities were discussed among themselves and decisions were taken by consensus. After the instrument was translated, it was sent to a third Brazilian speech language therapist, bilingual and resident in the United States, for back-translation into English. The initial version of the instrument and the back-translation were compared and the discrepancies were analyzed, discussed and defined by consensus. Results: the processes of translation and cultural adaptation required more effort in defining the terms of the used consistencies and did not change the structure of the original scale. Conclusion: the cultural equivalence of the Sistema de Classificação das Habilidades do Comer e Beber ­ EDACS-PT/BR was performed for Brazilian Portuguese.


Objetivo: llevar a cabo la traducción y equivalencia cultural y lingüística al portugués brasileño del Eating and Drinking Ability Classification System (EDACS). Método: la EDACS fue traducida al portugués por dos logopedas bilingües y especialistas en disfagia. Las dos traducciones se compararon entre los propios logopedas, discutiéndose las incompatibilidades y tomando decisiones por consenso. Una vez traducido el instrumento, se envió a un tercer logopeda, brasileño, bilingüe, residente en Estados Unidos para la retrotraducción al inglés. La versión inicial del instrumento y la retrotraducción se compararon entre sí, y las discrepancias fueron analizadas, discutidas y definidas por consenso. Resultados: los procesos de traducción y adaptación cultural requirieron un mayor esfuerzo en la definición de la nomenclatura de las consistencias utilizadas y no trajeron cambios en relación a la estructura de la escala original. Conclusión: se realizó la equivalencia cultural del Sistema de Classificação das Habilidades do Comer e Beber ­ EDACS-PT/BR para el portugués brasileño.

RESUMO

Building Bridges is an outreach initiative that has focused upon effectuating increased communication among biomedical scientists, educators, and students in Brazil and the United States-the countries with the greatest number of medical schools in the Americas. It has been a developing project over the last 6 years, and has involved faculty, students, and civic leaders from many universities in the two countries. This Special Issue of The Anatomical Record is a continuation of the Building Bridges initiative, and brings to the fore attention onto some of the creative research being done in biomedical science, evolutionary science, biomedical education, and current health topics in Brazil. Fostering open routes of communication among scientists is a core humanistic value that is at the heart of progress and is the center of our actions. This paper reviews the history of the Building Bridges Initiative.

RESUMO

This special issue of The Anatomical Record honors the bridges of knowledge and communications between Brazil and the United States in Anatomical Science, Healthcare, and Medical Education. The volume is organized in two sets of manuscripts: the first one stems from presentations of Brazilian colleagues in the latest iteration of the Building Bridges outreach initiative-a meeting entitled Building Bridges Back to Back, held at the Icahn School of Medicine at Mount Sinai, in December of 2019; the second, is a compilation of selected papers submitted independently to The Anatomical Record, authored by Brazilian researchers. All abstracts in this volume are featured in English and in Portuguese, and the opening piece is fully available in both languages. This bold and innovative addition represents the commitment of The Anatomical Record to support and help grow our bridges; it is a gesture of utmost respect and appreciation for our people and our culture. Together, our efforts to advance the knowledge in basic, translational, and clinical sciences, to expand our connections, and to build on our relationships across boundaries becomes a reality. This is an introduction and an overview of the manuscripts contained in this very special issue.

RESUMO

This commentary is a summary of two conversations among Brazilian former Minister of Health and oncologist Nelson Teich; Brazilian thoracic surgeon, Marcio Lucas; and anatomists Jeffrey Laitman and Daniella Curcio. We discuss a number of topics in Medical Education, Biomedical Science, and Healthcare, including the different stages of the Brazilian response to the COVID-19 pandemic. The discussions also explore: Brazil's and Brazilians strengths and struggles in the advancement of biomedicine; universal barriers to furthering advances; and problems specific to Brazil.

RESUMO Objetivo investigar o possível impacto da angulação do osso hioide na segurança da deglutição de pacientes submetidos à laringectomia supracricóidea. Métodos série de casos de 13 adultos, entre 48 e 79 anos, majoritariamente homens (n=11), submetidos à laringectomia supracricóidea em pós-operatório inferior ou igual a dez meses. Realizaram videofluoroscopia da deglutição de 5 ml de líquido fino, 5 ml de alimento pastoso e sólido, em livre oferta. A medida do ângulo do osso hioide foi definida por duas linhas: uma tangente à margem superior do corpo do osso hioide e uma tangente ao ponto mais inferior de sua margem inferior, paralela ao plano horizontal da imagem. O desfecho de aspiração durante o exame seguiu a escala desenvolvida por Rosenbek et al. (1996). Resultados Dos 13 pacientes, 5 apresentaram aspiração silente e 8 não apresentaram aspiração. Dos 5 indivíduos com aspiração, apenas 1 manteve preservadas ambas as cartilagens aritenoides em sua reconstrução e a angulação do osso hioide foi abaixo de 60º, em todos os casos. Dos 8 indivíduos sem aspiração laringotraqueal, a maioria (n=5) apresentava as duas cartilagens aritenoides em sua reconstrução e a angulação do osso hioide foi acima de 60º, em todos os casos. Conclusão uma angulação maior que 60º do osso hioide parece favorecer a proteção das vias aéreas inferiores e promover maior segurança do mecanismo de deglutição.


ABSTRACT Purpose to investigate the possible impact of hyoid bone angulation on swallowing safety in patients undergoing supracricoid laryngectomy. Methods the case series comprised 13 adults, between 48 and 79 years-old, male in its majority (n=11), within ten months or less post-supracricoid laryngectomy and cricohyoidoepiglottopexy. All volunteers were submitted to videofluroscopy at rest and during swallowing of 5 ml of thin fluid, 5 ml of pureed consistency and dry solid food. Images were captured in lateral view. The hyoid angle was taken at rest and defined by two lines: a tangent to the upper margin of the body of the hyoid bone and a horizontal line, tangent to the lowest point of its lower margin. The aspiration was assessed using the scale developed by Rosenbek et al. (1996). Results five cases had silent aspiration and eight had no aspiration. In the group with silent aspiration, only one individual had both arytenoid cartilages preserved, while all individuals had the hyoid bone angle below 60º. In the group without aspiration, five individuals had both cricoarytenoids preserved, while all cases had the average hyoid bone angle above 60º. Conclusion the hyoid bone being at an angle greater than 60º seemed to increase the protection of the lower airways, promoting a safer swallowing mechanism.

Immunohistochemistry is a specific and sensitive staining method for detection of several proteins. One important function of this method is to help us on the diagnosis of the presence of specific receptors as the estrogen receptors. The aim of this study was to compare two different methods to evaluate immunohistochemical staining intensity to detect the presence of ER in the nasal mucosa tissue: one using a digital system and the other through conventional direct microscopy observation. Sixty two stained samples were observed and analyzed under optic microscopy by three specialized professionals, who have graded intensity from: absent, mild, moderate and intense staining. Afterwards, an objective measurement was obtained by a relative optical density (ROD) reading, through the MCID 7.0 system of densitometric digital analysis (Inter Focus Imaging LTDA, Linton, England). Subjective and objective classifications were compared under statistical analysis (Kolmogorov-Smirnov and Spearman Correlation Test, p<0.05). We found a positive correlation between the subjective findings of observers and digital analysis in all the categories of beta receptor. For the alpha receptor, there was a correlation only in extreme categories. The subjective evaluations by observers and digital method by measuring the relative optical density show statistically significant correlations in quantifying the estrogen receptors by immunohistochemistry staining. This indicates that both methods show accuracy for the proposed study.


La inmunohistoquímica es un método de tinción específico para la detección de varias proteínas. Este método es importante en el diagnóstico de la presencia de receptores específicos, tales como los receptores de estrógeno. El objetivo de este estudio fue comparar dos métodos diferentes para evaluar la intensidad de la tinción inmunohistoquímica para detectar la presencia de RE en el tejido de la mucosa nasal: el primero utilizando un sistema digital y el segundo a través de la observación directa de microscopía convencional. Tres profesionales especializados observaron sesenta y dos muestras teñidas las que fueron analizadas con microscopio óptico con la siguiente intensidad de tinción: ausente, leve, moderada e intensa. Posteriormente, se obtuvo una medición objetiva a través de lectura de densidad óptica relativa (DOR) del sistema MCID 7.0 de análisis de densitometría digital (Inter Enfoque de imágenes LTDA, Linton, Inglaterra). Clasificaciones subjetivas y objetivas fueron comparadas bajo análisis estadístico (Kolmogorov-Smirnov y prueba de correlación de Spearman, p<0,05). Encontramos una correlación positiva entre los resultados subjetivos de los observadores y el análisis digital en todas las categorías de los receptores beta. Para el receptor alfa, hubo una correlación solamente en categorías extremas. Las evaluaciones subjetivas de los observadores y método digital midiendo la densidad óptica relativa muestran correlaciones estadísticamente significativas en la cuantificación de los receptores de estrógeno por tinción inmunohistoquímica. Por tanto ambos métodos demostraron ser correctos para el estudio propuesto.

RESUMO

This aim of this paper is to briefly report the education methodology employed at the Santa Casa School of Medical Sciences’ undergraduate program, founded in 1963, Sao Paulo, Brazil. Some relevant historical aspects are also brought out in order to provide readers with a glimpse of the social scenario that have yielded such successful model, based on integrative, practice-based strategies. Developed from simple concepts, innovative at that time, the educational system, the building structure of the hospital-university facilities and the pedagogical method remains modern and still inspires educators nowadays. The Department of Morphology is in alignment with these principles, working with the minimum possible lectures and exploring practical experience to the maximum. The disciplines of anatomy, histology and embryology comprise the curricular unit, elaborated per organ system, in a pedagogical program based on: timeline organization of the disciplines, strong theory-practice linkage, and a proactive learning system. The faculty members are trained in multiple areas to be as versatile as possible, delivering lectures and assisting in the practical training. New teaching methods may aggregate value to this model. We believe that such didactic approach can provide students with a realistic perspective of the main purpose and applicability of the basic sciences.

Multidisciplinary cooperation in health care requires a solid knowledge in the basic sciences for a common ground of communication. In speech pathology, these fundamentals improve the accuracy of descriptive diagnoses and support the development of new therapeutic techniques and strategies. The aim of this study is to briefly discuss the benefits of hands-on education on laryngeal anatomy and voice physiology in Brazilian graduate programs in speech pathology, as well as to describe a simple prototype that can be used as a useful educational tool for this purpose. The laryngeal anatomic support device was designed to provide a vertical frame to hold human or mammalian larynges with no preservation treatment, with the goal of allowing good visualization of the vocal folds during artificial phonation. The device was designed to provide the user the ability to manipulate the soft and cartilaginous structures of the larynx with near-natural biomechanical properties. The description of the project is detailed to allow the reproduction of this simple and inexpensive device. It may be used as an experimental feature in a variety of settings, from high-school programs to experimental research methods, and may suit a wide array of different educational models.

UNLABELLED: The vocal tract transfers its characteristics onto the sounds produced at the glottis, depending on its tridimensional configuration. AIM: this study aims to determine which of the seven oral vowels in Brazilian Portuguese is acoustically less impacted by changes to the vocal tract. MATERIALS AND METHOD: this is a cross-sectional prospective study. Twenty-three males and 23 females with ages ranging between 20 and 45 years (mean values of 28.95 and 29.79 years respectively) were enrolled in the study; none had voice complaints and their voices were normal under perceptive-auditory evaluation. Three-hundred and twenty-two sustained vocal emissions were digitized and acoustically analyzed by three computer programs combined. Results were compared against the distribution of resonance frequencies in a straight tube with one end sealed. RESULTS: statistical analysis showed that vowel /epsilon/ was significantly different when compared to the other vowels, with higher mean harmonic values and lower standard deviation for both genders. CONCLUSION: in Brazilian Portuguese, vowel /epsilon/ is less impacted by changes to the vocal tract and is significantly less attenuated in both genders. The inclusion of this vowel in voice assessment standard protocols may contribute to improve the quality of the information obtained as a result of quantitative spectrographic and acoustic tests.

O trato vocal transfere suas características ao som produzido na glote, de acordo com sua configuração tridimensional. OBJETIVO: Determinar qual das sete vogais orais do Português brasileiro sofre a menor interferência acústica das modificações do trato vocal. MATERIAL E MÉTODO: Estudo transversal prospectivo. Os indivíduos foram 23 homens e 23 mulheres, na faixa etária entre 20 e 45 anos (médias de 28,95 e 29,79 respectivamente), sem queixas vocais e com qualidade vocal normal na avaliação perceptivo-auditiva. 322 emissões vocais sustentadas foram digitalizadas e analisadas acusticamente utilizando-se a combinação de três programas computadorizados. Os resultados foram comparados à distribuição das frequências de ressonância de um tubo reto fechado em uma das extremidades. RESULTADOS: A análise estatística mostrou que a vogal /ε/ apresentou diferença significativa em comparação com as demais vogais, com valores harmônicos médios mais altos e menores desvios-padrão para ambos os sexos. CONCLUSÃO: Para o Português brasileiro, a vogal /ε/ sofre menor influência das modificações do trato vocal e apresenta atenuação significativamente menor para ambos os sexos. A inclusão dessa vogal nos protocolos padronizados de avaliação vocal pode contribuir para a qualidade da informação obtida por meio das análises espectrográficas e acústica quantitativa.

INTRODUCTION: Tracheostomized cancer patients require a special nursing care involving specifically related procedures. The goal of this study was to evaluate the results of a brief theoretical education program on tracheostomized patients care for nursing personnel. MATERIALS AND METHODS: One hundred and ten professionals have participated in this before and after-intervention one group study, and the program was carried out in a Brazilian hospital, a national reference center in oncology treatment. The questionnaire outcomes were compared to assess the theoretical knowledge level of the participants. RESULTS AND DISCUSSION: Statistical analysis showed that this program was efficient to enhance knowledge skills on specific nursing procedures related to care of the traqueostomized cancer patients. The low costs and simple structure required in the program may encourage the spread of similar practices to improve quality of nursing assistance in the hospital environment.
